All of the cords are attached correctly, all of the sudden it just stopped working. It's been working fine since Feb! It turns on, but it's nothing but a black screen.
Any suggestions? And will they replace mine if it's screwed up?
This is a discussion on Converter box was working, now black screen within the Converter Boxes and ATSC Tuners forums, part of the Over-the-Air (Antenna TV) category.

Two of the analog stations here (2 & 4) have been running shows about converter boxes and they list the manufacturers & their phone numbers. You could try calling the manufacturer. Maybe they have customer service.
It's not the number on the box, right? This is beyond annoying.
Before you give up, try unplugging it for 5 minutes, then see if it comes back on. You may have to scan again.

Thanks, but there was no luck.
I officially hate DTV.
There is a list of customer service numbers for the boxes on the gov site.

I would be interested to know which make and model died on you.

Insignia/Zenith or Dish DTVPal Plus. they're (to me) the only converters worth their salt. the Apex is last in the list because of its limited features (and static noise when any On Screen Menu is open or display is selected) but it does a great job holding a signal below 25% without pixelating.
