How do I get a return box for an unused receiver? - DirecTV

I called over two weeks ago to ask for a return box for a receiver that was the wrong one. The next technician installed the correct one, but said he was unable to take the other one back with him and I would have to call and ask for a box to be sent. This was on November 15th. I called and talked to a woman who took my information and said she would have one sent, but we still have not received it two weeks later. The receiver id is 0376 8835 4010. Could we please get a return box, because I now see that we have the monthly cost of this receiver added to our bill. Thank you very much,
